Form Download, Instructions & Samples

 

 

WELCOME!  On this page, you can download your very own wellness diary forms which will help you to keep track of what you eat and drink, how much you exercise, how you feel after eating certain foods, and setting goals (the fun part!).   

 

A great way to track your progress along the road to optimum wellness is to do a wellness diary for seven consecutive days, then after a few months, do this exercise again, and compare the results.

 

The file to download is in .pdf format (for greater compatibility) and is in four pages: 

2.)  Blank Wellness Diary Form which you can either print from the file each time, or print one copy and use as a photocopy "original".

3.)  A sample diary page showing the one-day totals of fat, fiber and caloric amounts of foods from the Standard American Diet (S.A.D.).

4.)  A sample diary page showing the one-day totals of fat, fiber and caloric amounts of foods from a HEALTHY diet.  Makes a very interesting comparison!  (This is the one you might want to consider as a goal to work toward.)

When you are ready, please click the graphic to the left to download the four-page .pdf document.   Be sure to "save to disk" and click "SAVE AS" and name the file "My Wellness Diary" and place it in the folder where you will remember for easy reference.

read your Bible every day, talk to God in prayer daily, regularly attend a church that preaches about Christ
